Online Course Application

In order to provide your child with the most effective tools to enhance their learning, North Carolina Wen Hua Chinese School (NCWHCS) may use online courses and applications, which would require parental permission for your child to obtain access to the internet. When registering or using such courses and applications, certain personal identifying information, including but not limited to student’s name, address, phone number, age, gender, grade, email address, and learning progress, may be provided to the web site operators. This form, when completed and on file with NCWHCS, will constitute consent for our school to provide such personal identifying information for your child, conduct online courses, and use online applications.
